What goes in each field
- Asset identity
- Name, year, make, model, and VIN or serial number as needed.
- Completion date
- The date the work was completed, written as YYYY-MM-DD.
- Mileage or hours
- The reading recorded when the work was completed.
- Unit
- Write miles, km, or hours beside the reading.
- Completed work
- Only work that was actually completed.
- Parts and fluids
- Names, part numbers, and quantities from the job record.
- Provider or DIY
- The shop or person who performed the work.
- Cost
- The supported amount for this record, without counting it twice.
- Currency
- The currency used for the recorded cost.
- Evidence filenames
- Receipt, invoice, photo, or document filenames kept with this record.
- Manual or source reference
- Document title, version, page, or link supporting the follow-up.
- Next due signal
- A date, mileage, or hour reading from the applicable source. Keep combined signals together.
- Open status
- Unfinished recommendations, questions, or pending work, kept separate from completed work.
- Correction reference
- The earlier record and reason for a correction; retain the original evidence.

Keep mileage, hours, and dates with their source
Record the reading and its unit at the time of the job. Copy any next due signal from the applicable manual or job record and keep its reference. This blank log supplies no service intervals.
If a source gives two limits, keep both due signals. After shop work, connect the paperwork to one owner-side record. For a modification, use the project change record to preserve the before and after details.
